Get With The Guidelines® - 360° Overview

Heart Failure (HF) is a major and growing public health problem, affecting over 5.7 million adults in the United States (1). Additionally, over 550,000 patients are diagnosed with HF for the first time each year and the disorder is the primary reason for 12 to 15 million office visits and 6.5 million hospital days annually (2).  

The American Heart Association will lead Get With The Guidelines®- 360° is a 2 year, 3-market quality improvement collaborative focused on transforming the heart failure system of care in metropolitan Chicago, Milwaukee, and St. Louis.

GWTG-360° will work to develop pathways that bring together all components of multidisciplinary patient care into a smoothly integrated system and reinforce use of evidence-based guidelines through AHA’s Patient Management Tool. 

Participating hospitals will have access to tailor-made patient and professional resources, receive enhanced one-on-one quality improvement consulting, and be able to participate in exclusive webinars, podcasts, and in-person meetings to share best practices at both a local and 3-market level.

The Initiative will utilize AHA’s premiere quality improvement program, Get with the Guidelines® – Heart Failure, to measure heart failure-specific demographic, clinical, and outcome data and evaluate temporal trends across the continuum of care.
