Mission: Lifeline Stroke Wyoming works with hospitals, emergency medical services, rehabilitation facilities and health care leaders across the state to enhance stroke preparedness, strengthen systems of care and improve access to timely treatment and rehabilitation.

Post-acute facilities will work to enhance guideline-based post-acute care for stroke patients across Wyoming by helping facilities adopt the American Heart Association’s Post-Acute Stroke Program Standards—addressing patient care guidelines, coordination of care and timing of assessments and education.
