Join us for a kick-off webinar to learn about opportunities for post-acute stroke care facilities.

Post-acute facilities will work to enhance guideline-based post-acute care for stroke patients across Wyoming by helping facilities adopt the American Heart Association’s Post-Acute Stroke Program Standards—addressing patient care guidelines, coordination of care and timing of assessments and education.
