Advancing Million Hearts®

Heart Disease and Stroke Prevention Partners Working Together

The American Heart Association launched the Advancing Million Hearts® program in 2016 to connect the AHA, state health departments, quality improvement organizations and local partners working to decrease heart disease and strokes in their states and communities. Partners collaborate to develop unified strategies that build upon existing efforts and further the Million Hearts goals.

Since 2016, AHA and Million Hearts® Collaboration partners have worked with approximately 338 organizations in ten states. The 2019 participating states are listed below along with a summary of the event and you can link to meeting information from 2016-2018 as well.
