We’re seeking sustainable solutions that address adverse Social Determinants of Health (SDOH) with the goal of generating long-term positive health outcomes. This strategic approach is designed to achieve two objectives—capital access for those who have little or no other opportunities for funding and positive health impact by addressing structural barriers to create opportunities for being healthy. The SDOH as we define them are:

We’re seeking applicants for the American Heart Association’s Social Impact Funds who are addressing social determinants of health challenges impacting residents in the following locations.

We fund sustainable solutions focused on access to health care, housing, and affordable and healthy food; economic empowerment; or closing the educational achievement gap in under-resourced communities. Applicants may be non-profit organizations or for-profit social enterprises.
