SEQIP Webinar - Stroke Continuum of Care - Great Rivers Affiliate

The Stroke Encounter Quality Improvement Project - or SEQIP - will be presenting a webinar series in the Fall of 2018 entitled:  Navigating the Stroke Continuum of Care.  

Presented by the University of Kentucky, this program will address: 

• Stroke Transitions of Care:  Assessments
• Preparing the Stroke Patient for Acute Care Discharge
• Best Practices for Patient/Caregiver Education

Note:  These lectures will be accredited for nurses and social workers.
