IMPLEMENT- Lipid Management™

Despite advances in evidence-based therapies, barriers to optimal lipid management exist across the care continuum, including workflow complexity, prescribing challenges, referral pathways, prior authorization requirements, patient communication and therapeutic escalation. This initiative is designed to identify those barriers and help health care teams develop practical, scalable solutions that can improve care delivery in real-world cardiology and primary care settings.

To help address these challenges, the American Heart Association has launched
IMPLEMENT-Lipid Management, a three-year quality improvement initiative designed to improve lipid management care in cardiology and primary care settings through scalable, patient-centered models of care. The initiative supports healthcare organizations in identifying, testing, and implementing practical strategies that strengthen lipid management workflows and improve care delivery in real-world clinical practice.

What is IMPLEMENT-Lipid Management?

IMPLEMENT-Lipid Management will convene a diverse cohort of cardiology and primary care teams in a national learning collaborative focused on identifying effective care pathways and evidence-based therapy escalation models, sharing successful approaches and accelerating adoption of best practices. By bringing together multidisciplinary experts from across the country, the initiative aims to transform insights into practical improvements that can benefit clinicians, clinics and patients.

The initiative is focused on developing and scaling practical strategies that healthcare professionals can implement within existing clinical workflows. Through national education, resource development and dissemination of successful models, the initiative seeks to support broader adoption of approaches that can strengthen lipid management efforts across cardiology and primary care settings.

Participating Healthcare Clinics

The American Heart Association will work with healthcare professionals from 10 healthcare clinics across the country using a learning collaborative approach to understand clinical workflows and barriers to care, amplify successful models of care nationally, and support the Association's mission to be a relentless force for a world of longer, healthier lives.
