What is CKM Center Certification?
A new designation for delivering integrated, person-centered CKM care. CKM Center Certification recognizes health care sites that demonstrate a commitment to coordinated, evidence-based care across specialties—including primary care, cardiology, endocrinology, nephrology, obesity medicine, and more.
Built on the American Heart Association’s rigorous certification framework, this program is currently in the feasibility testing phase with a small cohort of early adopters, representing sites across the CKM care continuum.

American Heart Association CKM Health Initiative
CKM center certification is part of the American Heart Association CKM Health Initiative, a multi-year effort to transform care delivery for people with, or at risk for, cardiovascular, kidney and metabolic conditions. This initiative also includes a national implementation science program with 150+ participating sites, patient and community engagement strategies, and professional education campaigns to equip care teams with the latest science.
