About the Initiative

Launched in 2024, the Cardiovascular-Kidney-Metabolic (CKM) Health Initiative represents a holistic, patient-centered approach to bridging gaps in fragmented care practices for people with a cluster of conditions known as CKM syndrome. CKM syndrome is made up of heart disease, kidney disease, diabetes and obesity, and leads to poor health outcomes. Information, tools and resources on CKM health care are available for professionals to begin implementing insights today.
For health care organizations in target locations, the AHA is offering participation in a new implementation science project to help build insights, test practices, and measure outcomes over the 4-year initiative.
Introducing CKM Health Groundbreaker Sites
Eligibility
Presently, implementation science teams are recruiting participation from health care organizations in target geographies, each selected based on health-related factors including health disparities, obesity trends, and population demographics. Health care organizations in the below areas are invited to participate in the CKM Health Initiative as CKM Groundbreaker sites.
