The Cardiovascular-Kidney-Metabolic (CKM) Center Alliance brings together leading health care organizations committed to delivering integrated, team-based care for patients with cardiovascular, kidney and metabolic conditions.
The Alliance is part of the American Heart Association’s broader effort to improve care for CKM patients, working alongside other programs and resources to support health systems in building, refining and leading coordinated cardiovascular-kidney-metabolic patient care. Originally established as the Cardiometabolic Center Alliance, this work represents some of the earliest and most experienced programs in integrated cardiometabolic care and continues to expand in reach and impact.
