The American Heart Association (AHA) Connected Care™, Powered by Cadence, a remote care delivery system that will provide heart and cardiometabolic care to patients at home following an acute hospital stay.

Connected Care aims to shorten hospital stays and prevent avoidable readmissions by connecting heart failure patients with high-quality care and remote patient monitoring support.

This program is a covered benefit for Medicare patients and there is no cost to hospitals.
