The American Heart Association and Providence's Health Equity Hypertension Project are working collectively to support faith- and community-based organizations in Los Angeles in reducing uncontrolled high blood pressure.

Home blood pressure monitoring may be especially useful for:

  • Anyone diagnosed with high blood pressure (HBP or hypertension).
  • Individuals starting high blood pressure treatment to determine its effectiveness.
  • People requiring closer monitoring, especially individuals with risk factors for high blood pressure and/or conditions  related to high blood pressure.
  • Pregnant women, experiencing pregnancy-induced hypertension and/or preeclampsia.

Train-the-Trainer Virtual Sessions

Do you want to learn how to deliver AHA’s EmPOWERED to Serve virtual health lesson program and how to share and teach these lessons within your organization or faith community?

Learn how to:

  • Identify a Health Champion to represent your organization (one or more participants can participate per organization).
  • Secure any necessary approvals within your organization regarding congregation and community participation.
  • Health Champion(s) completes train-the-trainer workshops.

During the sessions, you will learn to implement the EmPOWERED to Serve curriculum with your community members:

  • Health Champion provides educational resources to workshop participants.
  • Connect/refer participants to EmPOWERED to Serve workshops.
  • Follow up with participants on self-monitoring blood pressure.
  • Participant commitment: approximately two hours a week/approximately eight weeks
  • Health Champion commitment: approximately 3-4 hours/week
