The American Heart Association CKM Health Initiative™ harnesses collaboration between science and systems, clinicians and communities, data and decisions. Together, we are adapting care to meet the needs of patients with multiple diseases, supporting an integrated, intelligent model and focusing on outcomes that matter.

CKM Health is grounded in evidence-based science — connecting groundbreaking research to real-world clinical decisions. The initiative translates scientific discovery into practice by:
