2. Implement system improvements to meet the Target: Stroke goal.
3. Submit your application for the Target: Stroke Honor Roll.  (Reminder: The baseline survey must be completed in its entirety for a site to be eligible for Target: Stroke Honor Roll)
4. Share your success with your team, your community and with other hospital teams.
