This webinar will review important updates to the Heart Failure Patient Management Tool™* (PMT) including:
- New data elements and measures to support the newly released Advanced Certification in Heart Failure Measures. This includes the addition of two new measures to Get With The Guidelines:
- Care Transition Record Transmitted
- Advance Directive Executed
- Other new measures, including:
- Beta Blocker Medication at Discharge - Provides a breakdown (histogram) of the percent of patients discharged one each of the 11 Beta Blocker Medications tracked in Get With The Guidelines.
- Discharge Disposition - Provides a breakdown (histogram) of patients by discharge disposition.

Clyde W. Yancy, MD, MSc, FACC, FAHA, MACP
Dr. Yancy, a past president of the American Heart Association (AHA), the Magerstadt Professor of Medicine and Chief of the Division of Cardiology at the Northwestern University, Feinberg School of Medicine, of Chicago, IL. Additionally, Dr. Yancy is the Associate Director of the Bluhm Cardiovascular Institute at Northwestern Memorial Hospital.
Before his move to Chicago, Dr. Yancy was the Medical Director of the Baylor Heart and Vascular Institute and Former Chief of Cardiothoracic Transplantation at Baylor University Medical Center. Born and raised in Baton Rouge, LA, Yancy earned his medical degree from Tulane University School of Medicine and completed an internship and residency in internal medicine at Parkland Memorial Hospital in Dallas, TX. He completed a fellowship in cardiology at the University of Texas Southwestern Medical Center, which had been his professional home since 1989. At Southwestern, Yancy was a Professor of Medicine, Associate Dean of Clinical Affairs and directed the heart-transplant and heart-failure programs.
In addition to serving as a past president of the AHA, he was named the AHA Physician of the Year in 2003. He has said previously that his time with the AHA "permanently imprinted" on him the importance of population health. Yancy is also a fellow of the American College of Cardiology (ACC) and a master of the American College of Physicians. Dr. Yancy Chaired the ACC/AHA guideline writing committee for chronic heart failure and is a member of the Patient Centered Outcomes Research Institute as member of the Methodology Committee. Additionally he chairs the FDA Cardiovascular Device Panel and serves on several ad hoc and oversight NIH committees. He remains an active heart failure investigator and sits on the Data Safety and Monitoring Board for several major ongoing clinical investigations.
Win Kao is a Project Manager at Outcome, A Quintiles Company. He is responsible for the Get With The Guidelines® Patient Management Tool®. Win joined Outcome in 2004 as a member of the Help Desk, and since then has managed the Stroke, Heart Failure, and the Resuscitation modules. Win studied Biomedical Engineering and received a B.S. from Duke University and a M.S. from the University of Virginia in this area.
